In July 2014, Southboro Medical Group partnered with Advocates Inc. to incorporate a Pediatric Home Health project providing clinician and case management services to their Framingham office. Reliant Foundation’s goal is to work with current and prospective donors to ensure that the Southboro program will continue, and to bring the successful program model to an additional location at Reliant Medical Group.
Pediatric Home Health outcomes to date have demonstrated that this initiative improves the health and well-being of families with children facing complex challenges through services that provide access to the spectrum of medical, behavioral health, social, and community services needed. The pediatricians involved feel strongly that this project significantly enhances their practice.
To help provide this care in the future, our Foundation seeks to raise funds on behalf of the pediatrics department to cover part of the cost of running the program, beyond what insurance covers. Your support will bring the following key resources to our sites:
- Language support materials
- Social work support – both case management and short term therapy intervention
- Sufficient staff and a clinical case manager to manage patients and their potentially increased complexity
